What is Five Four Health?
Five Four Health is a Dallas based LLC that exists to provide teachers, healthcare professionals, first responders and veterans opportunities to invest in their physical, mental and social health.
For Businesses:
Who can join?
Local businesses dedicated to supporting teachers, healthcare professionals, first responders and veterans.
Are there any fees?
Joining our community is 100% free.
Who has access to our discounts and complimentary services?
Teachers, healthcare professionals, veterans and first responders. You have the option to promote your discounts to other participating businesses, but this is not required.
How does it work?
Create a business profile here. Once created, you can advertise any complimentary or discounted service you want to offer our members. You have full control of any offers you post. Offers can be set for a limited time, ongoing, redeemable one time, or multiple.
How do I join?
Click here to join our community. You will need a logo, business description and a complimentary or discounted service to offer our members (% off, special pricing, weekly promotion, etc).
How will I know who is a member?
Members have a digital Five Four Health membership card and their work ID linked to their profile. All members must upload their work ID when joining.
For Teachers, Healthcare Professionals, First Responders and Veterans:
Who can join?
Our platform is exclusively for teachers, healthcare professionals, first responders and veterans. You will need to submit a picture of your work ID as proof of employment.
Veterans can submit a military ID, VA issued ID card, DD214 or any other document providing proof of service.
How much are memberships?
Our first 1,000 members receive a two-year membership for the price of one. A yearly membership is $100 USD. All proceeds go towards expanding our vision of providing opportunities for local heroes to invest in their physical, mental and social health.
How does it work?
After joining, you will have access to discounts and complimentary services at local businesses.
Each business has complete control over what they offer and will specify how to redeem discounts and services.
